You will receive a confirmation email and be directed to payment if this form was successfully submitted. If you do not receive a confirmation email, then your form was not received. Please go back and try again.

Your child is not registered for the upcoming school year until this form is complete and payment is received.

Once your child has been successfully enrolled you will need to come to the  center to fill out addtional forms.  

In the event that all of our programs are filled you will not be able to use this form.  Please call so that you can be placed on our waiting list.
